Affordable Homes & Communities (AHC) recently received a 2026 Thome Aging Well grant to strengthen onsite health and wellness services at Charter House, a 55+ affordable apartment community in Silver Spring, Maryland. The $180,000 grant from Enterprise Community Partners (Enterprise) invests in a new wellness space at the community and the staff who will run it. AHC was one of 24 organizations awarded a combined $3.8 million in the final round of Thome Aging Well grants.
The year-long project will convert an under-utilized space at Charter House into a dedicated wellness and Resident Services area, with new furnishings and exam space. The expanded programming responds to a rise in emergency medical calls at the community tied to falls, strokes, and unmanaged chronic conditions.
Over the grant year, AHC’s Resident Services will reach 100 residents through screenings, care coordination, and quarterly partner-led workshops on fall prevention, mobility, medication awareness, and safe aging in place, and will complete wellness and safety assessments with 40 residents.
This complete initiative is funded by Enterprise through two grants, through the Thome Aging Well program and through the Section 4 Capacity Building for Community Development and Affordable Housing program, a Department of Housing and Urban Development program administered by Enterprise.

Launched in 2022 by Enterprise, the Thome Aging Well program uses a trust-based philanthropic approach that allows local organizations to respond to community needs quickly and directly.
“These organizations will have the flexibility to meet older adults where they are and respond to the needs they see emerging in their communities,” said Stephany De Scisciolo, Vice President of Impact, Evaluation, and Population Health, Enterprise. “This is how we ensure that these grants will have the greatest impact—by placing them in the hands of trusted, community-based partners working every day to ensure older adults can remain in their homes and continue to live safely and comfortably in the neighborhoods they know.”
To date, the Thome Aging Well program has awarded more than $23 million to 66 community-based, government, and Tribal organizations and reached more than 14,000 older adults through aging-in-place home modifications, trainings and workforce supports, preventive health screenings, food access and delivery programs, and additional services.
AHC is grateful for this generous investment creating a fully operational, onsite health and wellness suite at Charter House integrated into AHC’s Resident Services model, strengthening the support available to seniors and reinforcing their ability to age in place safely.
